Has anyone seen an entry form for the vic 150 and how can I get one?

Just bringing this back up to see if anyone has word when the entry forms will be out yet? Or if not, could some one give me some contact details for Sue Raphael or any of the other ladies that run this tourney every year.

Entry forms for Vic 150 will be at Adelaide Womens cup. Some of the girls from Keon Park are bringing them over (Carly and tracey) Although entry form is missing some relevent details. Like Handicap etc but at least its out there
