Youth Masters / Youth Cup Entry Form

Does anyone know where i can get a copy of the entry form for the Youth Masters or Youth Cup (whatever it's called) in January.

Its only a month or so away, i thought they'd be out by now.

If anyone can help, that would be great.

Entry forms have been sent to each state's Youth Team co-ordinaors and/or TBA State Managers.

We got ours almost 2 weeks ago.

If you're in Bankstown for the Youth Teams Challenge you can also place your entry in the tournament room during the week.
